NC Man Faces Insurance Fraud Charge Over Fake Doctor’s Note

RALEIGH, N.C. – North Carolina Insurance Commissioner Mike Causey announced Sept. 16 that Henry Lee Clyburn Jr., 42, of Charlotte, was charged with felony insurance fraud and common law uttering, a misdemeanor.

Investigators with the Department of Insurance Criminal Investigations Division accuse Clyburn of submitting a false doctor’s note to Progressive Insurance Company claiming he had been advised to miss work because of injuries sustained in an automobile accident.

According to the criminal summons, Clyburn presented the fake doctor’s note to Progressive to obtain reimbursement for the missed workdays and did not see the doctor identified on the note.

The offense occurred on May 19. Clyburn was served with a criminal summons on Sept. 14. He is scheduled to appear in Mecklenburg County District Court on Oct. 9.

Commissioner Causey encourages North Carolinians to help keep insurance premiums low by reporting suspicious fraud.

“Insurance fraud is not a victimless crime,” Commissioner Causey said. “It hits all of us in the pocket through higher premiums.”
